How to use this diary
Choose any seven days. Add a few notes when convenient; estimates and blank fields are fine. Use the date of the morning you woke up so an overnight entry is easier to follow. Print a blank copy if you prefer paper. This is an observation aid, not a diagnostic test or a treatment plan.
